Preview CY 2026 HHVBP Annual Performance Reports Are Now Available

CMS has released the Preview Calendar Year (CY) 2026 Annual Performance Reports (APRs) for the expanded Home Health Value-Based Purchasing (HHVBP) Model.

These reports give home health agencies an early look at the performance results that will determine their HHVBP payment adjustment for CY 2027.

As always, Simple continues to monitor HHVBP and CMS reporting updates to help home health agencies stay informed and prepared.

What CMS Announced

The Preview CY 2026 APRs are now available in iQIES.

The reports reflect agency performance during CY 2025 and include the Annual Payment Percentage (APP) that will be applied to Medicare Fee-for-Service claims with through dates in CY 2027.

The APR also provides information on several components used to calculate an agency’s payment adjustment, including:

  • Performance measure scores
  • Improvement Points
  • Achievement Points
  • Care Points
  • Total Performance Score (TPS)
  • Annual Payment Percentage (APP)

The CY 2027 payment adjustment can range from -5% to +5% based on an agency’s performance under the expanded HHVBP Model.

            What Measures Are Included

The CY 2026 APR is the first annual performance report under the expanded HHVBP Model to use the CY 2025 measure set.

That measure set includes 10 measures across OASIS, claims and HHCAHPS data.

            OASIS-Based Measures

            • Improvement in Dyspnea
            • Improvement in Management of Oral Medications
            • Discharge Function Score

                Claims-Based Measures

                • Discharge to Community–Post Acute Care
                • Home Health Within-Stay Potentially Preventable Hospitalizations

                  HHCAHPS Survey-Based Measures

                  • Care of Patients
                  • Communication Between Providers and Patients
                  • Specific Care Issues
                  • Overall Rating of Home Health Care
                  • Willingness to Recommend the Agency

For most measures, performance is based on CY 2025 data. The Discharge to Community–Post Acute Care measure uses a two-year performance period covering CY 2024 and CY 2025.

                          Why These Reports Matter

The Preview APR gives agencies an opportunity to review the data and calculations that will ultimately affect their CY 2027 Medicare payment adjustment.

This is an important time for teams to review not only the final APP, but also the individual measures and scoring components behind it.

Agencies may want to look closely at:

                          • Their Total Performance Score
                          • Improvement and Achievement Points
                          • Individual measure performance
                          • Their projected Annual Payment Percentage
                          • Any unexpected differences between the APR and internal performance data

Understanding where the score comes from can help teams identify which measures are contributing most to their overall HHVBP performance and where additional review may be needed.

Important Recalculation Deadline: September 4, 2026

If an agency believes there is an error in its Preview APR, CMS allows 15 calendar days from publication to submit a recalculation request.

For the CY 2026 APRs, the deadline to submit a recalculation request is:  September 4, 2026

CMS notes that recalculation requests apply to potential errors in the report or calculation. They do not apply to errors in an agency’s original data submission.

Agencies submitting a request should identify the specific information they believe is inaccurate and provide supporting documentation as appropriate. CMS also instructs agencies not to include protected health information (PHI) or personally identifiable information (PII).

                                  What Comes Next

The Preview APR is the first of three versions of the annual report.

CMS expects to release the Preliminary APRs on October 1, 2026. Agencies that submitted a recalculation request and disagree with the decision may then request reconsideration.

The reconsideration deadline is October 16, 2026.

If an agency continues to disagree following reconsideration, a request for Administrator review may be submitted by November 5, 2026.

Final APRs will be made available after the review process is complete and no later than 30 calendar days before the CY 2027 payment adjustment takes effect on January 1, 2027.

                                  How to Access Your Report

CMS makes HHVBP Annual Performance Reports available to authorized users through iQIES in the HHA Provider Preview Reports folder. Agencies with more than one CMS Certification Number will receive a separate report for each CCN.

For Simple customers, accessing the report is even easier: simply go to the CMS Reports tab in Simple to view your available CMS reports.

What Home Health Agencies Should Do Now

With the Preview APRs available, agencies should take time to review their results before the recalculation window closes.

                                  Consider:

                                  • Confirming your APP and Total Performance Score
                                  • Reviewing performance across each HHVBP measure
                                  • Comparing the CMS results with internal performance data
                                  • Investigating any unexpected discrepancies
                                  • Determining whether a recalculation request may be appropriate
                                  • Making sure the appropriate team members understand what the results may mean for CY 2027 reimbursement

The APR is more than a payment adjustment notice. It provides another opportunity to understand how your agency performed across the measures that make up HHVBP and where continued quality improvement efforts may have the greatest impact.

Simple Will Continue Monitoring HHVBP Updates

Simple will continue tracking CMS updates related to the expanded HHVBP Model, including upcoming Preliminary and Final APRs, to help home health agencies understand what is changing and how it may impact quality, reporting and reimbursement.
