CMS Releases July 2026 HHVBP Interim Performance Reports
CMS has published the Preliminary July 2026 Interim Performance Reports for the expanded Home Health Value-Based Purchasing Model in iQIES.
These quarterly reports give home health agencies an updated view of their performance during the current performance year, including cohort assignment, measure-level data and an interim Total Performance Score.
As always, Simple continues to monitor regulatory updates like these to help home health agencies stay informed and prepared.
What the July 2026 IPR Includes
The July 2026 Interim Performance Report includes:
- The agency’s assigned HHVBP cohort
- Performance data for applicable quality measures
- Measure-specific reporting periods
- The agency’s interim Total Performance Score
- A preview of performance under the CY 2026 applicable measure set
Agencies can use the report to compare their performance with other HHAs in the same cohort and identify areas that may require closer review before final performance results are issued.
Which Agencies Receive a Report?
An HHA receives a July 2026 IPR when it:
- Was Medicare certified before January 1, 2025
- Meets the minimum data threshold for at least one quality measure included in the quarterly reporting period

Preview of the CY 2026 Measure Set
The October 2026 IPR will be the first report to calculate improvement, achievement and care points using the CY 2026 applicable measure set.
To help agencies prepare, CMS has been gradually adding CY 2026 information to quarterly IPRs. The July 2026 report includes a CY 2026 Performance tab that previews current performance for several measures that will become part of the updated measure set.
The preview includes performance scores for:
- Improvement in Bathing
- Improvement in Upper Body Dressing
- Improvement in Lower Body Dressing
CMS noted that CY 2024 and CY 2025 data needed to calculate Medicare Spending per Beneficiary–Post Acute Care performance are not yet available. As a result, MSPB-PAC performance scores are not included in the current preview.
Some agencies may also not yet have an assigned MSPB-PAC improvement threshold if they were previously ineligible or did not meet the minimum data requirements for an earlier baseline period. CMS expects to assign those thresholds when the necessary data become available.
No action is required from agencies related to these missing preview values. Updated performance information will appear in future reports.
How to Access the Report
Authorized iQIES users can access IPRs through the HHA Provider Preview Reports folder.
To locate the report:
- Log in to iQIES.
- Select My Reports from the Reports menu.
- Open the HHA Provider Preview Reports folder.
- Select the applicable HHVBP report file.
Reports are organized by CMS Certification Number. Organizations operating under multiple CCNs should receive a separate report for each eligible CCN.
The reports are not publicly available and can only be viewed by users authorized to access the agency’s reports.
For SimpleConnect™ for Home Health clients: CMS reports are delivered directly to the SimpleConnect dashboard, giving teams a more streamlined way to access and manage important reporting information without navigating multiple systems.
Review the Preliminary Report Carefully
The Preliminary IPR gives agencies an opportunity to request a recalculation when they believe a measure or interim performance score was calculated incorrectly.
For the July 2026 report, recalculation requests must be submitted by July 31, 2026.
A recalculation request should include:
- The provider’s name, service address and CCN
- The specific data or calculation believed to be incorrect
- Contact information for an agency representative
- Supporting documentation that does not include protected health information or personally identifiable information
CMS asks agencies to submit only one CCN per request. Agencies comparing the IPR with an internal or vendor-generated report should also consider including that report and identifying whether the internal values are risk-adjusted and which data sources were used.
Recalculation requests should address potential calculation discrepancies. They do not apply to errors in the agency’s original data submission.
What Home Health Agencies Should Do Next
Home health teams should consider reviewing the report across clinical, quality and operational functions rather than treating it as a standalone score.
Important next steps may include:
- Confirming the agency’s cohort assignment
- Reviewing measure-level performance and minimum data thresholds
- Comparing current results with internal or vendor-generated reports
- Examining the CY 2026 performance preview
- Documenting any potential calculation discrepancies
- Submitting a recalculation request by July 31 when appropriate
- Watching future IPRs for updated MSPB-PAC data
Even when a recalculation request is not needed, the report can help agencies better understand current performance trends and prepare for the CY 2026 measure changes that will begin appearing in October.
